# Request Form Builder

The Request Form tab allows you to build your Request Forms and make edits as needed.

## Build Request Form

A default set of fields are included in the form builder. You can remove any field from the request form by clicking on the trash icon to the right of the field name. The **email address** field is a system field and always required in your form.

To add a new field:

1. Click the ‘Add Field’ button at the bottom of the form.
2. Enter a Field Label which will be the field name displayed to the Requestor.
3. Specify the Field Type
4. Indicate whether the field will be required by clicking on the **Required** checkbox below the Field Label.
5. Click ‘Save Schema’ to add you new field to the form.

## Charge Fee for Request

If your agency charges a fee to fulfill a request, make sure that the ‘Show the payment/fee notice on the Review & Submit step’ is enable.
